Since 2015, The Sontag Foundation has been an active member of the St. Johns County Health Leadership Council (HLC). The HLC is one of 12 collaboratives nationally recognized for role model status in displaying highly successful, multi-sector partnerships which focus on improving community health. The mission of the Council is to "promote, protect, and improve the health of all people in St. Johns County, FL."

Every three years the Council uses the nationally recognized, evidence based, strategic planning process called Mobilizing for Action through Planning and Partnerships (MAPP) for improving the health of the county. Council members collect data from national, state, and county organizations, as well as from community members and partners to better understand the current and future health needs of the community.

Results from this planning process result in the triennial community health assessment and community health improvement planning cycle.
