Since the grant review process has been completed for 2013 funding in our home community, the information provided on this page should be viewed as representative samples of documents typically requested by this foundation.
The forms downloadable from the links below are fillable pdf's in which data can be saved. Forms are updated annully, so earlier versions are
not accepted. It is advisable to first download an updated free version of Adobe Acrobat (8.16 or higher), then download and save the appropriate form to your computer before filling it out electronically. Each blank field can accept specified number of words, so be concise. Successful applicants have read the
instructions and the
FAQ to assure that the information needed to make funding decisions is submitted as requested. Documents must be saved with nomenclature specified in the instructions. Unfortunately, MAC users have experienced significant compatibility issues with fillable pdf forms. You will save time by completing your application on a pc.
